"New Document Set:" always showing while creating document set

Not sure why when I'm creating any document set first its show "New Document Set: Document Set name".

For Document Set I've created Document Set Name = "Document Set Name". Now when I'm using this created Document Set in My Document Library while creating its showing as Heading "New Document Set: Content Type". So always first "New Document Set:" is showing whenever I'm creating content type. Not sure why this is coming now. Please advice.
