Export Data to MS Excel

can someone help me on this pls?

I have an example that I would want to achieve when I apply Infopath in Sharepoint. Is this possible?

1. 
   

   Assume that there's a request form in Sharepoint with the following details:

   
   
   
   
   • Name = using textbox
   
   
   • Date of Birth = using date picker
   
   
   • Gender (Male or Female) = using radio/option button
   
   
   
   


2. 
   

   After the user filled out the request form, of course, he/she needs to submit/save it.

   
   


3. 
   

   Can the data from the request form exported in MS Excel with the following format?

   
   
   
   
   • Column 1: Name
   
   
   • Column 2: Date of Birth
   
   
   • Column 3: Male (1 if yes, 0 if no)
   
   
   • Column 4: Female (1 if yes, 0 if no)
   
   
   
   

Please advise.
