Save button not working in new input form

I made some code changes using SharePoint Designer 2013 and then added to a web-part. But it seems when I enter the data and click save to add a new record, nothing is happening and I am not sure why. I am attaching my code, if anyone can point me in the right direction I would appreciate it.
