List creation - Duel access

I am trying to create a form in sharepoint to be filled in by 2 departments, it requires 10 questions. The form is submitted by team 1 and the first 5 questions are completed and need to be required fields, this is then saved, the same form is then picked up by team 2 and the last 5 questions are to be completed but again these need to be required fields also and the form saved, is this possible?
